Insurance Information
We accept many insurance plans. Ask about our out-of-network and cash pay plans.
Appointments
Patients will be seen by appointment only, except in case of an emergency. Please call as far in advance as possible to make an appointment. If you cannot keep an appointment, please notify the office at least 24 hours in advance of your appointment time.
Fees & Payments
Our policy requires that all office visit charges be paid at the time of service. We accept cash, personal checks, MasterCard and Visa for payment of charges. You will receive a monthly statement itemizing charges and your current balance. Any arrangements for extended payments need to be addressed with our office. If your account is past due and you have not made payment arrangements with the business office, we will need to take additional collection measures.
Prescriptions & Renewals
Please request all prescriptions and authorizations for renewals between 8:30am - 5:00pm, Monday to Friday, when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances.
